Football & cheerleading and Fall Soccer registration for Monday thur Firday hours 7:00pm to 8:00pm cost 120.00 per child

• Registration Form
• Medical Form
• Hardship Form
• Birth Certificate
• Proof of School (Copy of a report card)
• Proof of Residency (Copy of utility bill)
• Small Non-returnable Photo
